0Who is the patient?
This tool uses NEWS2, which the Royal College of Physicians validated for acutely ill adults aged 16 and over. It is not validated in children or in pregnancy, and a score calculated on them is misleading rather than merely imprecise.
1Is there an immediate life threat?
ONSET question 1. Tick anything that is true now. If you tick one, stop reading this screen — call and start ABCDE.
2Is this a patient who should not wait?
ONSET question 2 — the high-risk features. These are the patients who deteriorate in the waiting area.
3Vital signs
Enter what you measured. Leave anything you did not measure blank — the tool will tell you the score is incomplete rather than pretend the missing value was normal.
Scale 2 is only for a patient with a documented target of 88–92%. Choosing it without that documentation makes a hypoxic patient look acceptable.
4Pain
Recorded because it is on the Al Shifa triage screen and because severe pain is a high-risk feature under ONSET question 2. It is not part of the NEWS2 score — adding it to an acuity score causes over-triage.
